The first thing you need to check is if you have permission to create new roles. Go to Users > Roles > Administrator (assuming you’re using the Administrator role). Under the “Users” tab, check that you have the create_roles (Create Roles) capability.
If you do indeed have that cap and it’s not working correctly, you likely have another plugin (or theme) that’s messing things up. What you’ll need to do from that point is to deactivate all other plugins to see if it corrects the issue.
I just realized you posted in the Members – Admin Access add-on forum. Are you saying this happens when you activate the Members – Admin Access plugin?
Under the “Users” menu in the admin, you should at least see these options:
All Users
Add New
Your Profile
Roles
Add New Role
Once you activate the Members plugin, the Roles and Add New Role items should appear. If they do not appear, go to Settings > Members and click the “Enable the role manager” checkbox.
When I try to create a new ROLE, it brings me to a page that says, “Sorry, you are not allowed to access this page.”

Thank you for your help. You’ve seemed to narrow my issue.
Under USERS, my only options are ALL USERS, ADD NEW and YOUR PROFILE.
I deactivated ALL plugins I still only see those options.
Even though it doesn’t seem to be an issue with MEMBERS, can you point me in a direction to troubleshoot?
